If you appoint a health and safety consultancy to assist you, then it’s very helpful to make sure there is a clear division of responsibilities. Our form will enable you to do this.

How to use the form

This form should be used at the beginning of your agreement with the consultancy. It will help to define what you would like them to carry out. It may also assist you in identifying gaps which you will need to fill by other means, e.g. in-house staff.

Tick “yes” or “no” against each statement and also tick “F/I” if there is further information to be included at the end of the form.
